New Software Reduces Hospital Readmissions


While most times hospitalization is necessary, evidence suggests that many readmissions are not. Not only can readmissions can be very costly, but they can also be harmful. In light of this, Predixion Software and Indiana Health Information Exchange (IHIE) have partnered together to offer a predictive solution that will reduce preventable hospital readmissions. The predictive health analytics solution will be offered to accountable care organizations (ACOs) and hospitals across Indiana.

In Indiana alone, nearly one in five Medicare patients returns to the hospital within a month of discharge. What these patients may not realize is that their readmission could have been completely avoided. According to the Journal of the American Medical Association, about 48 percent of hospital readmissions are preventable. The Centers for Medicaid and Medicare Services reported that 76 percent of readmissions within 30 days are preventable. ACOs and hospitals could save about $31 billion annually by preventing readmissions.

Predixion has already made its first module, Predixion Readmission Insight, available for healthcare organizations.  This will provide a risk stratification of its patients to determine proactive intervention opportunities, which will be available as a data feed or tablet-based module. Dr. Dick Roskam, the chief medical information officer for Franciscan Alliance, Inc., finds that better forward-looking data analytics tools like this are essential to healthcare organizations."To further improve both quality and cost outcomes, knowing what may happen to a particular patient or within a population of patients becomes paramount, compared to our traditional retrospective approach to data. Applying the science of predictive analytics to healthcare, especially when combined with Big Data, holds great promise for us,” said Roskam.

Curt Sellke, vice president of analytics for IHIE, also announced this partnership will be available to Indiana ACOs. "In conjunction with Predixion Software, we look forward to leveraging the INPC to develop and deploy predictive applications that enable physicians to reduce hospital readmissions, choose optimal therapies, reduce redundant testing and avoid drug-drug interactions, among other life-saving and efficiency-generating efforts."
