Hillcrest Chooses Phytel to Improve Patient Care and Communication



Hillcrest HealthCare System (HHS), a network of six hospitals and 49 clinics in northeast Oklahoma, has selected Phytel to help HHS' physician group execute its population health management strategy for chronic and preventive care, potentially leading to an accountable care organization (ACO) setup.

Phytel is a provider of automated, provider-led population health improvement, used to make sure patients receive recommended visits, tests, procedures and follow-up care through software that automatically tracks and proactively reaches out to patients.

Deploying Phytel Outreach, a system to keep providers and patients in touch, to 100 of HHS’ primary care physicians – nearly half of the 195 clinicians at Utica Park Clinic –will serve to ensure patients receive recommended preventive and chronic care.

Phytel’s outreach software has helped get more care to people at Hattiesburg Clinic in Mississippi, and boosted revenue by about $4 million, according to the company’s website.

HHS will also use Phytel Insight, a Web-based registry and advanced analytics tool that “measures the effectiveness of quality initiatives and generates actionable data to improve health outcomes across a population,” according to a recent statement.

As part of its efforts to improve and extend care, HHS also announced that 60 Utica Park Clinic physicians were recently accepted into Medicare's Comprehensive Primary Care Initiative (CPCI), which gives primary care physicians the resources they need to better coordinate care for Medicare patients.

"We'll use Phytel's solutions to help build the infrastructure around the CPCI program and other quality monitoring programs," said Jeffrey Galles, D.O., chief medical officer at Utica Park Clinic. "We can also use these tools to build additional infrastructure if we elect to move forward with an ACO.”

ACOs, the linchpin of healthcare reform, coordinate cooperation among providers to improve the quality of care for Medicare beneficiaries and others, and reduce unnecessary costs.
