CMO Selects ZeOmega Software to Enhance Healthcare Management Capabilities



ZeOmega Inc., a popular supplier of healthcare management software solutions, announced that a unit of Montefiore Medical Center in Bronx, N.Y., CMO, Montefiore Care Management has decided to use its Jiva care management software to improve patient engagement and coordination. Components of ZeOmega’s solutions include dashboards, care analytics, assessments and portals, and will be integrated into the CMO's care management programs.

"ZeOmega's offerings further support our approach to care management," says Jack Wolf, chief information officer, Montefiore Medical Center according to recent reports. He also added, "We selected ZeOmega after an extensive vendor search because advanced IT solutions like Jiva are an essential component of our integrated strategy for engaging all stakeholders in the care continuum."

CMO is an in-depth care management business that provides progressive interventions and services to help Bronx residents attain optimal health. CMO has more than 15 years of experience in facilitating patient-centered care, and is focused on enhancing clinical quality, improving the patients experience and lowering healthcare costs.

CMO cares for more than 225,000 health plan beneficiaries of federal and state healthcare programs and members. These include Pioneer Accountable Care Organization (ACO), which is one of 32 in the United States.

 "ZeOmega's Jiva product matches our care management processing requirements, and gives us the capability to build the additional advanced care management functionality required for ACOs, shared savings programs and other value-based purchasing arrangements to be successful," says Stephen Rosenthal, president and chief operating officer of CMO.

Jiva's new capabilities will enable Montefiore to assist in identifying member problems that require intervention by care managers and other providers, integrate disparate information into one location, and develop custom care plans. It also allows the distribution and customization of correspondence based on user specification and preference for the post discharge process. ZeOmega plans to demonstrate Jiva’s capabilities to help ACOs tackle care requirements this year at the HIMSS Conference at booth #6252.
