PatientPoint and Cornerstone Working Together on Patient Engagement


PatientPoint, a provider of care coordination solutions to drive engagement with patients and healthcare staff, today announced that Cornerstone Health Care will use its platform to make it easier for patients to register and be discharged, have medication managed and be surveyed for satisfaction, and potentially have better outcomes, while also streamlining front office and back office services to improve workflow, according to a release.

Cornerstone, an independent physicians group, serves more than 80 locations in North Carolina.

According to a 2009 study, coordinated care “patients have an 88 percent reduced risk of dying of a cardiac-related cause when enrolled within 90 days of a heart attack, compared to those not in the program,” Dave Chase reported, referencing the study.  And, “clinical care teams reduced overall mortality by 76 percent and cardiac mortality by 73 percent,” he added.

With the PatientPoint solution, physicians will now be able to review patient data quickly and easily at the point of care, and within their already established workflow, rather than having to leave their workflow to find and review the data manually, the release stated.

PatientPoint will deploy electronic point-of-care check-in/check-out devices to strengthen administrative processes, perform medication management, administer screenings and assessments for identified patients, and offer patient satisfaction surveys.

The company will also help Cornerstone develop clinical workflows for its care team members within patient-centered medical homes (PCMHs) and accountable care organizations (ACOs) “by delivering content directly through its patient monitoring dashboard and to the group’s electronic medical records (EMRs),” the release reported.

“As we advance our PCMH and ACO models, we aim to strengthen the physician-patient engagement to improve clinical outcomes and manage costs,” said John Walker, MD, CPE, chief medical officer of Cornerstone, in a statement. “We see PatientPoint as a tool that not only allows us to enhance our patients’ experience, but also one that augments our existing population health tools by creating a delivery infrastructure to message patients at the point of care. We believe this will help patients become more actively involved in managing their conditions more effectively and create more meaningful conversations with their physicians.”

PatientPoint will use its technology to help Cornerstone move toward the ACO model under the Medicare Shared Savings Program. Through this program, Cornerstone will work with CMS to provide Medicare fee-for-service beneficiaries with quality of care, “while reducing the growth in Medicare expenditures through enhanced care coordination,” the release explained.
