Crystal Run Healthcare Enters Partnership with AMC Health to Improve Care for Patients


To improve care coordination and outcomes for patients with chronic conditions, Crystal Run Healthcare revealed that it has entered into an agreement with AMC Health. A NY-based multispecialty group, Crystal Run boasts more than 300 providers and is one of the fastest growing physician groups in the country.

In order to enhance care for Crystal Run patients, under the agreement, AMC Health will provide solutions for heart failure, diabetes, chronic obstructive pulmonary disease and other chronic illnesses. To enable Crystal Run to eliminate and avoid gaps in care that occur after patients are discharged from a hospital, it will also provide remote patient monitoring services.

Jonathan Nasser, MD, co-chief clinical transformation officer at Crystal Run Healthcare, stated that AMC Health was selected because of its proven and scalable tele-health solutions that significantly reduce unnecessary resource utilization, particularly preventable 30-day readmissions.

"By identifying care gaps and providing services and tools that support seamless and effective care transitions, AMC Health will make it easier for our doctors to deliver high-quality, cost-effective and efficient evidence-based care,” he added.

A post-hospital discharge service will provided by AMC Health along with tailored disease-specific programs employing biometric monitoring, healthcare coaching and logistical services for in-home monitoring.

Nesim Bildirici, president and CEO of AMC Health, explained that by bringing together all healthcare elements under one roof and creating new efficiencies that result in better health management and outcomes, the company is pleased to be selected by Crystal Run in order to support its powerful and innovative model of care under the accountable care organization (ACO) framework.

He added that Crystal Run will gain access to real-time, clinically actionable data by leveraging its comprehensive tele-health offerings. It will furthermore help keep its patients out of the hospital by empowering its clinicians to detect pre-acute conditions earlier. By meeting the required quality benchmarks, its solutions will enable the company to maximize its shared savings.
