New Site Supports Coordinated Care and Patient Centered Medical Home Models

The Agency for Healthcare Research and Quality (AHRQ) has launched a Patient Centered Medical Home Website which focuses on providing information on the medical home for policymakers and researchers. According AHRQ,“The medical home model holds promise as a way to improve health care in America by transforming how primary care is organized and delivered. Building on the work of a large and growing community, the Agency for Healthcare Research and Quality (AHRQ) defines a medical home not simply as a place but as a model of the organization of primary care that delivers the core functions of primary health care.”

The following attributes are central of the medical home model:

*Patient-centered; the medical home is a care model that is relationship based and focused on the whole person. It actively supports helping patients and family members to manage and organize their own care.

*Comprehensive; meets the majority of each patient’s physical and mental health needs through a care team approach

*Coordinated care; the primary medical home coordinates care across all parts of the health systems on behalf of the patient

Responsive to patients needs and committed to quality and quality improvements 

The new website also provides access to a searchable database of publications and other resources on the medical home model and critical medical home issues. For more information visit the Agency for Healthcare Research and Quality.
