What do you do when you run a healthcare plan and most of your members are healthy? Some insurers are finding that it’s hard to stay in touch with these more elusive members.
The TriZetto Group, Inc. , announced today that CDPHP®, a nonprofit health plan serving nearly 400,000 members in upstate New York, has gone live on TriZetto’s care management software solution to put healthcare information at the fingertips of patients and health plan employees, healthy or not.
The nonprofit says Trizetto’s solutions support its population health management and member engagement initiatives.
Population health management are strategies that improve the systems and policies that affect health care quality, access, and outcomes, with the ultimate goal of enhancing the health of an entire population while saving costs. Another goal CDPHP is working on is engaging all its members to interact with the solution and other members through social media.
“TriZetto’s care management solution supports our move from disease and case management to a broader, even more impactful population health management approach,” Tracy Langlais, vice president of medical affairs operations, said in a press release. “CDPHP is helping all our members manage their health, not just those with chronic diseases. We’re a supportive partner to our members, and TriZetto’s solution enables us to walk the talk.”
Langlais explained that TriZetto’s applications, Clinical CareAdvance® and Personal CareAdvance®, help the nonprofit share helpful medical information and care tips with its healthy members.
“Unsupported, healthy members, in fact, may assume more health risks as years go by. But with many healthier than unhealthy members, ongoing communications with these individuals proved difficult. TriZetto’s solution stratifies our population and automates coordinated campaigns of secure, web-based notifications with members across the entire continuum of health,” said Langlais.
Trizetto’s platform enables care managers to focus more on care delivery than on questions about benefits. Even better, care managers now have a “complete member view for informed consultations,” according to Langlais. “The new platform helps members really engage,” she said. Members can get access to an online library of helpful information and resources.
Many health plans are turning to this type of approach with customers, finding it’s a way to make them stand out in this competitive business. Some health plans even offer personal health and lifestyle coaches to stay in touch with their healthy members.
The application also allows CDPHP payers to share insights with an employer about the health of its workforce, the results of educational and intervention programs, and opportunities for further improvements to population health.
